In-home long-acting injection support
Long-acting injection care, brought to you
Paragon helps eligible patients receive prescribed long-acting injections at home while coordinating with their existing care team. We regularly support patients in Omaha, eastern Nebraska, and southwest Iowa.
Submitting a request starts a review. It does not schedule an appointment or enroll anyone in services.
How it works
A review first, then coordinated next steps
Start with the appropriate form. Paragon will review the details, confirm availability, and explain what comes next.
- 1
Send a request or referral
Patients and caregivers can start with the patient request. Healthcare professionals can use the provider referral form.
- 2
The team reviews the details
Paragon reviews the information provided, then follows up about medication details, the service address, coordination needs, and next steps.
- 3
Care is coordinated
When the service can move forward, the team coordinates medication and injection logistics with the people involved in the patient's care before an in-home visit is scheduled.
Care that stays connected
Support built around the patient's existing care team
- At-home support
- For eligible patients, Mobile Injection Care provides an in-home option for receiving prescribed long-acting injections.
- Connected care teams
- Paragon works alongside existing healthcare providers and keeps the appropriate people informed as the request moves forward.
- Clear cost conversations
- Any associated service fees are discussed before care moves forward.
"Paragon Partners has been amazing to partner with. They have made it so easy for our clients to receive their injections in their own homes and have always provided us with timely documentation of these. This has made a significant impact on compliance with injections!"

Your local pharmacy
Paragon remains your local point of contact
Paragon is part of a connected pharmacy network. Depending on the patient's needs, other network locations may support intake, medication coordination, logistics, scheduling, or follow-up.
Pharmacy locations do not define or guarantee Mobile Injection Care coverage. Contact the team to confirm service availability for a specific address.
